Pregnancy on a plant-based diet
It can be done well and it requires more attention than at any other stage of life, with several nutrients where the stakes are genuinely high.

Major dietetic associations regard well-planned plant-based diets as appropriate during pregnancy and lactation.
The planning matters more here than at any other stage, and this article is an orientation rather than a substitute for professional input, which anyone pregnant on a plant-based diet should have.
The non-negotiables
Vitamin B12. Requirements increase in pregnancy. Deficiency in the mother affects the developing nervous system, and severe deficiency in infants of unsupplemented mothers has caused permanent harm.
Supplement, reliably, throughout pregnancy and breastfeeding.
Folate. Required before conception and in early pregnancy to reduce the risk of neural tube defects. Supplementation is recommended for all women planning pregnancy, plant-based or not, typically from before conception through the first trimester.
Plant-based diets tend to be higher in dietary folate, and the supplement is still recommended because the evidence for the reduction in risk is based on supplementation.
Iron. Requirements rise substantially. Iron deficiency in pregnancy is common in all dietary patterns and is associated with adverse outcomes.
Test rather than assume. Manage absorption carefully — vitamin C with meals, tea and coffee away from meals. Supplement if indicated, on medical advice.
Iodine. Requirements increase, and deficiency in pregnancy causes irreversible impairment of the child's cognitive development.
This is the nutrient most often missed when dairy comes off the plate, and the one where the consequences are most serious.
Supplement, or ensure reliable intake, and discuss the amount with a professional. Do not rely on seaweed, whose content is unpredictable and which can supply dangerous excess.
Vitamin D. Supplementation is recommended in pregnancy in many countries regardless of diet.
Omega-3, particularly DHA. Required for fetal brain and eye development. Conversion from plant ALA is limited.
An algal supplement is the reliable route and is worth taking.
Choline. Important for fetal brain development, and richest in eggs. Plant-based intakes tend to be lower. Worth discussing supplementation.
Calcium, for the baby's skeleton and the mother's bone stores.
Zinc, with requirements increased and absorption lower from plant sources.
Energy and protein
Energy requirements increase, particularly in the later stages, though less than the eating for two framing suggests.
Protein requirements increase too. With nausea, reflux and reduced stomach capacity, meeting them requires attention.
Practical measures: energy-dense foods, smaller more frequent meals, and soy foods which deliver protein efficiently.
The practical difficulties
Nausea and food aversion, which frequently make the carefully planned diet impossible for a period.
Advice during this phase: eat what you can keep down, take the supplements, and do not add stress about optimal nutrition during a period when you cannot eat.
Ginger has reasonable evidence for nausea in pregnancy and is discussed as an option in clinical guidance. Discuss with your midwife.
Severe persistent vomiting is a medical condition requiring treatment and is not something to manage with dietary tweaks.
Iron supplements causing constipation, which is already common in pregnancy. Alternate-day dosing has evidence for equal or better absorption with fewer side effects, and is worth discussing.
Reduced stomach capacity later on, requiring smaller and more frequent meals.
Foods to be careful about
Standard pregnancy advice applies and is worth knowing in a plant-based context.
Wash fruit and vegetables thoroughly. Avoid unpasteurised products. Be careful with pre-prepared salads and sprouted seeds, which carry a listeria and salmonella risk — sprouts are frequently recommended to be cooked rather than raw during pregnancy.
Limit caffeine according to local guidance.
Avoid alcohol.
Be cautious with vitamin A from supplements, since high doses are teratogenic — this is more relevant to liver and to some supplements than to plant foods, where beta-carotene is not a concern.
Be cautious with herbal preparations and teas, many of which have insufficient safety data in pregnancy.
Breastfeeding
Requirements remain elevated and in some cases increase further.
B12 is the critical one. The infant's supply comes from the mother's milk, and maternal deficiency produces infant deficiency with potentially serious consequences.
Continue supplementing throughout.
Iodine, DHA, vitamin D and energy also remain important.
Get professional input
Genuinely, and early.
Ask your midwife or GP for a referral to a dietitian. Ask for blood tests including B12, ferritin and vitamin D.
Take a prenatal supplement designed for or suitable for plant-based diets, and check what it contains rather than assuming it covers everything — many do not include iodine or DHA.
The reassurance
Well-planned plant-based pregnancies are common and produce healthy babies. Studies comparing outcomes have not found disadvantages where nutrition is adequate.
The risks come from inadequate planning, and the specific things needing attention are known, few and addressable.
Supplement properly, eat enough, get tested, and involve a professional.
Anyone pregnant, planning pregnancy or breastfeeding on a plant-based diet should consult a registered dietitian and their midwife or doctor. Do not take iron or high-dose supplements without medical advice.
